    Learn How to Use the NAIC Life Insurance Policy Locator  

    Every year, millions of dollars in life insurance benefits remain unclaimed. Sometimes, beneficiaries can't find their deceased loved ones' policies or, in some cases, may not even know the policies exist. To help solve this issue, the NAIC created the Life Insurance Policy Locator. The Life Insurance Policy Locator is a free online tool that helps consumers find their deceased loved one’s life insurance policies and annuity contracts.

    Steps You Need to Take  

    The locator is easy to use. In your web browser, navigate to naic.org, hover over Consumer, and click Life Insurance Policy Locator under Tools.

    Create an account by entering your email address and name.

    Create a password.

    Login and agree to the process.

    Enter your name and address.

    Submit a search request by entering the Deceased’s information from the death certificate:

    Social security number

    Legal first name Legal last name Date of birth Date of death

    After all required fields have been filled, click on the submit button.  

    Your request will be stored in a secure, encrypted database where participating life insurance and annuity companies can access the information through a secure portal. You will receive a ”Do Not Reply” email confirming the request details you submitted. If a policy is found and you are the beneficiary, the life insurance or annuity company will contact you directly, usually within 90 days.

    If no policy is found or you are not the beneficiary, you will not be contacted. Please note that the NAIC has no policy or beneficiary information.

    This form will help you determine if there was a life insurance policy in effect at the time of death of a loved one. When you submit the requested information, we will search our records to find out if you are the beneficiary for any unclaimed State Farm individual life insurance policies.

    If you are the Beneficiary After we receive your request for a search, we will review our records of individual life policies. If we find a policy for which the deceased was the insured or owner and you are the beneficiary, we will contact you within 30 days of your request and provide you with paperwork to process any death proceeds due.If you are not the Beneficiary We can only provide information about life insurance policies to insureds, owners, and beneficiaries. If you are not the beneficiary of record we cannot provide you any information about a policy. However, State Farm Life will attempt to locate and contact the beneficiary listed on the policy record to begin the life claim process.

    Your name and contact information

    Your relationship to the insured

    Insured person's full name and nickname (if any)

    Insured person's last known address or last residence state

    Insured person's dates of birth and death

    Insured person's Social Security Number or last 4 (if you have it)

    All fields are required unless noted as optional.
